All these questions ensure I have everything I need to be able to book you in quickly. If you are looking to book in back to back appointments for you and a friend/family member, please include ALL their info in this application alongside your info as well.

If you are looking to book MULTIPLE tattoo designs, you can put them all in one application. As a heads up, if the form does not allow for you to attach a photo (too large, too many etc), feel free to email me your reference photos. My email address is deviousniktattoos@gmail.com

Absolutely NO Minors, NO Face/Neck Tattoos, Cover ups and Tattoo Re-works are done on case-by-case basis . 

Saturdays are ALWAYS limited , so please include a weekday if possible so I can get you in sooner. 

Since there is a high demand for tattoo time with me, I may have to turn down projects that do not suit my abilities or interests. I appreciate each and every one of you that have supported me through this journey 😈
